Terms and Conditions for Nutrition 4 Nippers

​

In this document the terms ‘We’ and ‘Therapist’s’ refer to Denise King, State registered Dietitian and International Board Certified Lactation Consultant, and Clare Whitehead, RGN, HV, International Board Certified Lactation Consultant. The term ‘parent/carer’ and ‘you’ refers to the person signing this agreement.

 

Initial assessment

  • It is the parent/carer's responsibility to read our 'terms and conditions' document and 'privacy information' document prior to booking your session. By proceeding with your booking at Nutrition 4 Nippers you are agreeing to the terms and conditions within both documents. Once the relevant session is agreed between the parent/carer and clinician, the parent/carer will receive an email or text confirming the details of your child’s first appointment including the date, time, location, and fee.

  • The initial assessment will ideally be a face to face contact and take place at your home (this may differ according to local COVID-19 restrictions at the time of your booking). The session usually lasts between 60-120 minutes. Time will be allocated for the clinician within your session allowance for, the recording of notes, writing up a personal care plan, and any relevant correspondence.

  • Your therapist will always let you know whether they have the correct skills and experience to meet your child’s needs and will signpost you to other professionals where necessary.

Travel

  • All fees are inclusive of travel within 5 miles of the therapists house. Any additional mileage will be charged at 45p per mile. There will also be additional charges for any toll fees.

Payment

  • All monies must be paid in full prior to the session beginning. Payment can be accepted through bank transfer or cash. For bank transfers please use your invoice number as the payee reference, bank details are listed on your invoice. A written receipt can be provided on request.

  • Fees for additional reports, meetings or visits will be invoiced once booked and must be paid in full prior to the work being completed.

  • We will always seek your agreement prior to undertaking any additional work that will incur further fees.

  • Clients using private health insurance are responsible for settling the invoice with Nutrition 4 Nippers and then claiming from the company concerned. It is advisable to check the level of your cover before agreeing to therapy.

Fee changes

  • Fees are subject to annual increases from 1 April each year.

  • Current clients will be given 8 weeks’ notice of any changes in fees.

Cancellation

  • If you need to cancel an appointment please contact Nutrition 4 Nippers as soon as possible, any cancellations under 24 hours before will be charged at the full session rate, there will be no charge if the cancellation is more than 24 hours in advance and a new appointment will be organised as soon as possible.

Non-attendance

  • The full session fee will apply in the event of non-attendance. Non-attendance includes:

    • If you are not in when the therapist comes to an appointment at your home.

    • If you do not attend the private setting as agreed

Working hours

  • Our working hours vary and can be adapted to meet individual needs.

​

NHS services

  • Your child may be eligible for infant/childhood feeding advice within your local NHS trust. If your child is currently seeing or waiting to be seen by an NHS therapist, we will need to be informed at booking. The NHS therapist should also be informed of Nutrition 4 Nippers involvement to ensure effective treatment.

Medico-legal work and tribunals

  • We are unable to offer medico-legal or tribunal work at this time.

 

Safeguarding

  • Denise King & Clare Whitehead both have a DBS enhanced disclosure.

  • Should any safeguarding concerns arise, Nutrition 4 Nippers has a legal obligation to share information with relevant professionals in line with the Safeguarding Children's Act 2004.

Protecting your information

  • We are registered with the Information Commissioner’s Office (ICO) as Data Controller’s.

  • All client details, case notes and correspondence will be treated confidentially and stored securely in line with the General Data Protection Regulations and the Data Protection Act 1998.

  • Information is stored on a secure electronic system.

  • Reports and programmes are password protected.

  • Confidential information is stored securely in accordance with General Data Protection Regulations and the Data Protection Act 1988.

  • In accordance with law, all records will be kept securely until your child is 25 years old. After this time all records relating to your child will be destroyed.

  • You may apply in writing to access a copy of your child’s notes or to request modifications of any inaccuracies. These requests will be dealt with within 30 days.

Complaints

  • If you are unhappy with any aspect of the service provided by Nutrition 4 nippers please contact us directly to discuss your concerns.

Electronic communication

  • Email is not a 100% secure method of communication. With your consent, it will be used for correspondence and to send letters, reports and other relevant documents.

  • Documents will be password protected and saved in Printed Document Format (PDF).

  • Correspondence via email to other professionals will be copied to you as necessary.

  • We will refer to your child in emails by their initials only.
